There is a $65.00 application fee (non-refundable) to be included and returned with your completed application. Failure to include deposit will delay your application approval. 

All members are required to pay their annual membership fee of $75.00. You will be invoiced for your first annual fee upon acceptance of your application and confirmation of your official membership. 
Part of your application fee will be used for a background check including possible criminal records.

Agreement 

I acknowledge and affirm that the information provided by me in this Application, including all attachments and exhibits, is true and correct to the best of my knowledge. I understand that if I am granted general membership of receive credentials of any kind from Amazing Grace Ministries Evangelistic Association Inc. My membership and my credentials may be withdrawn or terminated by Amazing Grace Ministries Evangelistic Association Inc. At any time and without notice, if any information is false or misleading. 

I hereby authorize Amazing Grace Ministries Evangelistic Association Inc. and it's agents to conduct a complete investigation of my background, character, reputation, and fitness, before granting me general membership in Amazing Grace Ministries Evangelistic Association Inc. or affording me any credentials of certification. 

This Application shall constitute authority to all of my past and present employers, to all educational institutions I have attended, to all religious institutions and other organizations to which I have been associated, to all government entities (including criminal records check), and to any other person or entity having information about me, to fully disclose such information to Amazing Grace Ministries Evangelistic Association Inc. Such information includes, but shall not be limited to personal files and records, grades, and grade point averages and transcripts, earned degrees, professional licenses, attendance and discipline records, complaints, suspensions, license revocation or fines, and such other oral and written information which in the exclusive discretion of Amazing Grace Ministries Evangelistic Association Inc. may be relevant to determining my suitability for general membership and credentials of any kind. 

I hereby release any such employer, educational or religious institution, government entity, and other person or entity, and their representatives or agents, from any claim or liability of any kind for complying with such information requests of Amazing Grace Ministries Evangelistic Association Inc.

CODE OF ETHICS 
Each member of Amazing Grace Ministries Evangelistic Association Inc. will agree to the following ethical standards in all aspects of life and ministry.

I will: 

• Maintain high standards of moral, financial and business integrity in all my life and ministries in order that the work of the Lord not be blamed;.
• Actively pursue relationships and fellowship with the other ministries and churches within my locality, state, and nation, in accordance with the commands of Holy Scripture;
• Respect each person's differences in doctrine, style, and personality as they follow the Lord Jesus;
• Maintain a teachable spirit, and a willingness to learn from other leaders in the Fellowship;
• Speak only in an edifying manner regarding other ministers and ministries of the Body of Christ;
• Have no part, directly or indirectly, with a church split;
• Not establish nor seek to establish any other ministry within five (5) miles of the location of any church in which I have served as a member, volunteer staff or paid staff unless I have permission in writing from the senior pastor. I will have no part whatsoever in attracting or seeking to attract any member or attendee of my previous church to be a part of my new ministry;
• Support and give my loyalty to any pastor under whom I serve. If I cannot provide this commitment of support and loyalty, I will advise the pastor of that fact stating my reason(s) in a Christ-like manner and withdraw from the Fellowship and seek to affiliate with another local church in which I can join in good conscience;
• Follow the process as commanded in Scripture (Matthew 18:15-20 and I Cor. 6:1-8) when faced with a brother or sister in violation of this code or other biblical or moral standards;

General Requirements For Membership 
You...

• have accepted Jesus Christ as your personal Lord and Savior.
• accept the Bible as written and undivided as your guide for living and are in agreement with the Amazing Grace Ministries Evangelistic Association Inc. Statement of Faith and have signed and returned the form.
• accept and agree with the Amazing Grace Ministries Evangelistic Association Inc. code of ethics and have signed and returned the form.
• have a spiritual call to Christian ministry and service.
• commit yourself to a life of spiritual growth and service.
• evidence your call to Christian ministry in your life, Bible study, and Christian service.
• will faithfully and consistently support Amazing Grace Ministries Evangelistic Association Inc. as led by the Spirit of God.

The credentials which we offer to our members are as follows: 

 Certified Christian Worker  

A Certified Christian Worker is a credential which is issued to lay persons who are called by God as helpers. Certified Christian Workers are not authorized to perform any sacerdotal services.

Specific Requirements for the Certified Christian Worker Credential: 

• Meet the general requirements for membership.
• Be recommended to be a Christian Worker by your pastor or another pastor of an established congregation
• Be recommended to be a Christian Worker by your peers in Christian ministry.
• Other criteria as determined by the Credentials Committee.
• We may suggest you need to complete our Correspondence Bible School Course for full acceptance.

Licensed Minister  

A Licensed Minister is a credential which is provided to persons who have little or no ministry experience. Such persons are authorized to preach and to assist a senior pastor or other staff pastors in any way that they can, in order to begin to get some exposure to Christian ministry. However, they are not authorized to perform any sacerdotal services. We believe unless in exceptional circumstances exist these credentials should be given by your local church.  If however the circustance warrant other needs then will we consider your applicaton

Specific Requirements for the Licensed Minister : 

• Meet the general requirements for membership.
• Be recommended to be Commissioned to Preach by your pastor or another pastor of an established congregation.
• Demonstrate a level of biblical and theological proficiency
• Be recommended to be Commissioned to Preach by your peers in Christian ministry.
• Other criteria as determined by the Credentials Committee.
• You need to complete our correspondence Bible School Course for full acceptance.

 Normally, a person will serve at least two years as a licensed ministers credential before being considered for next position of ordination credential in the ministry track offered by Amazing Grace Ministries Evangelistic Association Inc.

 Ordained Minister 

 An Ordained Minister's credential is provided to those persons who have, an established and proven ministry Such persons are not only authorized to perform sacerdotal services, but are also required to be performing such services on a regular ongoing basis. We believe unless in exceptional circumstances exist these credentials should be given by your local church.  If however the circustance warrant other needs then will we consider your applicaton

Specific Requirements for the Ordained Minister Credential: 

• Meet the general requirements for membership.
• Be recommended for ordination by your pastor, church elders or other approved religious leaders.
• Be recommended by your peers in Christian ministry.
• Demonstrate a level of biblical and theological proficiency.
• Have an established and proven ministry (this may include a transfer of ordination from another recognized church, fellowship, or denomination).
• Be performing sacerdotal services on a regular or ongoing basis.
• Other criteria as determined by the Credentials Committee.
• You need to complete our correspondence Bible School Course for full acceptance
